Each person enrolled in the program is assigned a supports coordinator who assesses the individual’s needs and helps to locate, coordinate, and monitor appropriate services to meet those needs. Examples of services are employment support, respite, supports for independent living, residential programs, daytime activities, adaptive equipment, and architectural and vehicular adaptations.
